How to navigate these guidelines

These guidelines were developed as part of the Integrating Health Care Planning for Health and Prosperity in North Queensland Project, also known as the place-based health planning project. The guidelines describe overarching principles of place-based health planning and key steps including detailed procedures. The key elements of this approach include:

  • gaining a deep understanding of the context in which services are accessed and delivered, including community dynamics, diversity, historical influences, and geographic considerations
  • identifying and engaging critical partners at all levels who are essential to the success of the initiative
  • collaboratively identifying community health priorities and service needs
  • co-designing or redesigning service models with genuine, purposeful community engagement
  • implementing, monitoring, and evaluating new or adapted models of care.

The guidelines are presented in the following chapters:

  • Chapter 1 provides a brief overview of the project and outcomes from phase one which informed the work in communities.
  • Chapter 2 discusses how to define a place for the purposes of place-based health planning.
  • Chapter 3 outlines the core principles underpinning participatory, place-based health service planning. These principles are fundamental to the planning process, highlighting key factors for consideration and providing a foundation for the process across various applications.
  • Chapter 4 adopts a procedural focus, detailing the essential steps involved in implementing participatory, place-based health service planning. This chapter is supported by practical examples and suggested activities to guide application.
  • Chapter 5 discusses the resources required to undertake this type of project.
  • Chapter 6 outlines the next steps to progress planning work.

After discussion of the guidelines, practical examples are provided to illustrate their application. These include examples of resources, templates, and case examples drawn from our work across the three project communities. All materials and templates developed during the project are available for use and adaptation with appropriate acknowledgement. Individuals or those within organisations interested in place-based health planning can use this guide either as a step-by-step roadmap or to consult specific sections for targeted information and support.
